Completion of this form DOES NOT guarantee you a room for your event. You will receive an email confirming your request. All facility reservations are subject to cancellation should an unexpected Church-related event require the scheduled facility.

Facility Use Covenant

This is a covenant between EUMC and I, , as the Event Leader/Coordinator for agree to be present during the time that the group uses the facility (including the entire time the doors are scheduled to be unlocked) and I agree to pay for any damages to any property of Ebenezer United Methodist Church that is incurred because of neglect or abuse.
