Parents' Night Out

Parents’ Night Out  (Hosted by KidzCare) 
Date & Time: November 21, 5:00–8:00 PM 
Location: Ebenezer KidzCare (in the Gym/Children’s Wing)  
Who: Children registered with KidzCare (please register each child)  

What’s Included  Supervised activities, games, and crafts 

Cheese/pepperoni pizza dinner 

Safe, friendly environment staffed by KidzCare team 

What to Bring  Water bottle labeled with your child’s name 

Any needed comfort items (optional) 

Cost  One child: $25 

Two children: $40 

Three or more children (same family): $50 

Payment and registration required to reserve your spot. Space is limited. 

Please complete one form per family and list all children attending. Include allergy/dietary info in the form notes. 

Check-In & Pick-Up  Check-In: 4:50–5:10 PM at the KidzCare desk 

Pick-Up: 8:00 PM with photo ID 

Notes & Policies  Pizza is provided; if your child has food allergies, please send an alternative dinner. 

All children must remain on campus for the full event time unless picked up by a listed guardian.
